The operative note is the doctors summary of the tubal ligation operation. It usually tells us what type of ligation was done and a few specifics on how he performed it. The pathology report describes what was found in the tissue (tubal segment) which was removed from the patient’s body and how much. They like both because they can get more information that way. Not sure about the pathology report and not the operative report though. I requested the operative report and the pathology report, but I got a call from the medical records department today and the lady told me that for some reason the Doctor who did my TL did not dictate it! So she could not obtain the operative report. Any input would be appreciated
