Hi Shell, I'd love to help you, but I do not know which insurances pay for TR. It's my understanding that just about most isurances offer a policy that covers TR, but it's up to your employer if they chose to offer that policy to their employees. It's a highter premium so, most employers don't offer TR coverage as part of their policies. You would have to contact your isurance provider to see if your policy covers TR. Hope that helps.
What did you do to retreive that information? I have BCBS of DC PPO. Would I just call them up or have a pre approval form filled out.. What is the code for the procedure?

I also have bc/bs of fl and it does not cover TR.... it depends on the policy and different employers choose different benefits to offer to their employee's.

I sorta of got in writing, they already sent me a form to send in for reimbursement and it says right in our book, either one tubal reversal or one steralization per person in a lifetime. So, I will take the reversal. YEAH!!
