Thank you, I am still working on getting funds gathered. I am so looking forward to just having my life back. Though more children would be a blessing, just knowing that I could live with out so much pain would be great. Would we have to file a claim with our insurance company after the procedure? and do we have to get the forms for this?. thanks for additional information. Also, I have a fibroid tumor, but OB/ doctor says I can still have tubal reversal since it is not a major problem. Is this usually the case with fibroid? I know hormones plays a role in getting them and they can happen to any one.

We will give you the codes for the insurance company but we do not itemize the costs of the procedure since it is a discounted all inclusive fee. The diagnosis code (ICD-9) that insurance companies use for bilateral tubal occlusion is 628.2. The procedure code (CPT) for tubal anastomosis is 58750.

we have anthem for state of ohio, how would we process a break down claim. I am thinking this would happen after the procedure. and the nurse or office could help us break down each claim form to process. How would we code it, under both that is listed on site? any ideas would help.
