It did not take long at all. With me being there they had me sign for a copy then they went and pulled/ copied one for me while I waited. I then turned around took it to a faxing place and sent it right to CHTRC. I wish you all the best. I have sent a request to the hospital that performed my sugery two weeks ago, is this normal? Can Chapel Hill just fax the request if I mail them my permission form? Thanks a bunch.
