You get a receipt with the procedure codes on it (in case your insurance covers TR)and the amount you paid. You will not get an itemized receipt..ie how much the US was, anesthesia, surgery etc. It is not broken down, it's all inclusive.

I already know that my insurance wont cover it. I have a printed letter from FSA stating that the tubal reversal is an eligible expense. Did they give you a receipt at CHTRC? FSA said I have to submit a receipt with the claim form.

with mine, I never prepaid for something, but was able to use it all way before I had paid it all into it. Basically, once the bill is submitted into the Insurance company, your out of pocket is paid from your FSA, and they send you a statement out telling you how much was paid, and how much you have left to "spend" in your account.
As far as it being a covered expense, I don't know. I would research it again (double check it, get the persons name who said yes it would be, etc) My Insurance company doesn't cover TR's because it is elective to repair your tubes, so I'm not sure if my FSA would have covered it (I didn't do FSA the year I had my TR)

I have a question about the flex spending account. I am going to use it for a reversal in 2010, but they wont let me pay for the services before I actually get them done. So, I am going to borrow the money from my dad and then pay him back when they reimburse me. I have to give them a detailed receipt and they will deposit the funds into my bank account. I just wanted to hear other people's experience with their FSA. My FSA already told me that tubal reversal is an eligible expense, but I just have a HUGE fear that they will not reimburse me and I will have to find another way to pay my dad back. Please anyone who can offer any input on their experience with FSA would be very helpful. I am waiting until I hear a little more about FSA to schedule a date for my reversal.
