No, we do not recommend an HSG prior to the reversal since this test is not helpful in predicting the outcome of the reversal. HSG testing is used to assess the interior of the uterus and to determine if the tubes are blocked. If you have had a tubal ligation, we know your tubes are blocked!
With HSG testing, dye is injected through the cervix and into the uterus. Once the uterus has filled, the dye will flow through the tubes. If a woman has blocked tubes due to a tubal ligation, the dye can only flow to the point where the tubes have been tied, cut, burned, or clamped. While this might allow the doctor to determine the amount of proximal tube remaining (the segment attached to the uterus), nothing is learned with regard to the amount of distal tube remaining (the tubal segment on the OTHER SIDE of the area where the tubes were blocked with the sterilization), nd that is very important information to have. Also, HSG does carry a risk of infection and it can be an expensive test to perform.
If a woman's records are not available, we do offer the option of starting with a screening laparoscopy so that the tubes can be assessed under direct visualization. Although there is an additional charge to add this procedure, a partial refund is given if it is found that the tubes are too short or too damaged to repair. Fortunately, Dr. Berger is able to repair the tubes in 98% of cases.
