I was told yes it was covered the first time, I called a second time and was told it was not covered. I called a third time and spoke to a supervisor and it is covered @ 80% out of network and 90% in-network and I have it in writing. It seems like you get different answer everytime you call an insurance co, but if you get a yes, make sure to ask for it in writing. Good luck!

I have BCBS and I got a refund of 1200 from them. It was worth sending the paperwork in. I believe they paid the Anesthesia and something else that was on the Invoice... To all of you that have BCBS do not call and ask they also told me no but I still got some of what I paid back!

Hi Linda~ Yes, I had my TR done Oct/2006. Still patiently hoping and praying for our BFP. TTC has been harder than we thought. I'm currently seeing a RE to give us some help. I've started injections and we will try IUI for the first time this month =)
Did you find out if your Insurance will cover any of the cost? I have BCBSAZ and they told us no but they do pay for all the blood test, US and fertility specialist visits, medication and a good portion of the injection meds! So we are grateful for that.

Linda if you pay out of pocket for the surgery some insurance companies will then let you file it as out of pocket and you can get some back. That is what we are doing.

Some insurance companies do cover part of the costs for tubal reversal surgery, but coverage is specific to your individual plan.
For patients interested in long-term payment arrangements, however, we offer a pre-payment plan or financing through MedChoice Financial. With the pre-payment option, patients can pre-pay the surgical fee without interest prior to the surgery date. To utilize this option, a non-refundable $250 fee is mailed to us at: Chapel Hill Tubal Reversal Center, 109 Conner Drive, Suite 2200, Chapel Hill, North Carolina 27514. At that point, an account is opened in your name and payments of any amount are made as you are able -- for as long as you wish. For the standard fee of $6900, you do not have to wait until the balance is paid to schedule surgery – you can schedule at any time with a $250 down payment. The balance is due at least three weeks prior to the surgery date. We are a fee for service practice and do not accept payments from insurance carriers. If your insurance carrier will cover part or all of the cost of your surgery, please be sure they send the reimbursement check to you directly. The insurance code (ICD-9) for bilateral tubal occlusion is 628.2. The procedure code (CPT) for tubotubal anastomosis is 58750. Any pre-certification or pre-authorization that may be required by insurance policies are the responsibility of the patient.
